Access & Choice Service NEW worker

We are pleased to announce that Capital Care now has an Access and Choice Community Support worker based in our practice - one day a week / completely free.

>> Emotional health and mental wellbeing
>> Family/whānau issues
>> Social interactions and engagement
>> Money matters
>> Housing issues
>> Managing drug use, drinking or gambling

Talk to your health provider (GP, practice nurse) and ask them to put you in touch with our Access and choice worker.
