Eligibility
1.Check government guidelines regarding New Zealand GP enrolments at : www.health.govt.nz
2.Ring reception or come into the practice to discuss eligibility (FYI: we take patients from the Hutt Valley and CCDHB area, and may have further eligibility criteria depending on current patient numbers).
3.If eligible, collect forms from the practice (photo ID and proof of 2+ year visa/citizenship required). Forms can be filled out at the practice or at home. Processing takes 5-10 working days
4.Before seeing a GP, you must have a fifteen minute first nurse appointment (cost $35), unless you are re-enrolling within 3 years, or are under fourteen.
